Kickstart Questionnaire

This is for young people to complete. Please send all Kickstart forms to Jayne Jackson, Football in the Community, Reebok Stadium, Burnden Way, Lostock, Bolton, BL6 6JW - email jjackson@bwfitc.co.uk

   

Kickstart Referral

Email this to Bob Horrocks. Referrals are considered at Kickstart Management Group Meetings which are held every month.

   

Kickstart Receipt or Invoice

Carers can use this form where activity providers do not have their own invoice or receipt. Kickstart must have some written evidence of expenditure.

   

Kickstart Evaluation

This is to be completed by the referrer and must be submitted to Jayne Jackson within three months of the activity starting. This is crucial for funding to continue in the future.

   

LASE Guidance

 
   

LASE Referral

Referrals to the LASE Scheme must be made by the Leaving Care Team. Please send the LASE Referral form to Eileen Terry or Heather Clarke, LASE Scheme Co-ordinator, Corporate Children's Office, Endeavour House. Email: eileen.terry@bolton.gov.uk or heather.clarke@bolton.gov.uk

   

Mentoring Scheme Referral

Please send the Mentoring Scheme Referral form to Joanne Duffy, LAC Mentoring Co-ordinator, Bolton Lads' and Girls' Club, 18 Spa Road, Bolton, BL1 4AG

   

Personal Education Allowance Referral

Please email the Personal Education Allowance (PEA) form to Debra Hurst - debra.hurst@bolton.gov.uk -and Lyn Hailwood - lyn.hailwood@bolton.gov.uk - who will allocate a time slot at the next Resource Management Panel.

How to buy a laptop for a young person

PEA money can be applied for to buy a laptop, if a young person you are working with needs one. £500 a year is each young person's educational allowance.

  1. Get a written quote from a computer supplier (excluding VAT, as the Council is exempt) 
  2. You will need a standard laptop, suitable for studying with 
  3. Student Microsoft Office Licence 
  4. Microsoft Office installed on the laptop 
  5. Internet Security Software (such as Norton) installed on the laptop 
  6. Laptop bag 
  7. DO NOT TELL THE SUPPLIER HOW MUCH YOU HAVE TO SPEND OR THEY WILL JUST SEND YOU A QUOTE FOR THAT AMOUNT! 
  8. If you are satisfied with the quote (you may have to check with the PEA Resource Panel) ask your administrator to raise an order, charging it to the PEA budget which Claire Mulvaney in Strategic Finance will give you.
